World Breastfeeding Week 2017
Distinguishing between intolerance and allergy
SMA H.A., SMA LF Lactose Free and SMA Wysoy – where can they be used?

Dear Healthcare Professional,

Welcome to the August edition of our eNewsletter aimed to keep you updated with the latest news in infant nutrition.
This week hosts World Breastfeeding Week so we would like to celebrate those breastfeeding mums and provide you with some breastfeeding resources that you may find useful, such as unbranded breastfeeding helpsheets!

We will also be discussing the difference between allergy and intolerance as we know this can be confusing and we would like to let you know how some of our specialist formulas could help in the management of these issues for those mums who are not exclusively breastfeeding.

Kind regards,

The SMA® Professional Team

World Breastfeeding Week 2017

Newborn

This week is World Breastfeeding Week which aims to encourage and support those mums who are breastfeeding. Breastfeeding is the best way to feed an infant as it provides both nutritive and non-nutritive benefits and adapts to a baby’s needs2. One of the benefits of breastfeeding is that it promotes the infants immune system.
You will find a number of breastfeeding resources to help you to support parents on the SMA Professional website. These include videos of mums giving advice and sharing their own experiences of breastfeeding, helpsheets for you to share with parents on a range of useful topics such as breastfeeding positions in 7 different languages and suggestions of other websites parents can visit to get further advice.

Distinguishing between intolerance and allergy

Newborn

There is often confusion between a food allergy and food intolerance, both of which can affect breast- and formula-fed infants. A food allergy occurs when the immune system reacts to a usually harmless food protein. This immune response leads to the release of substances, such as histamine, resulting in allergic symptoms, such as gut, skin and respiratory symptoms. The risk of an infant developing an allergy increases if there is a family history of allergy as shown in the infographic1. An example of a food allergy is cows' milk protein allergy (CMPA), which affects approximately 7% of formula or combination-fed infants, with the prevalence being highest during the first year of life3. It is the most frequent cause of food allergy among infants and young children globally4.

Contrastingly, a food intolerance does not involve the immune system, and may be enzymatic or pharmacological. Lactose intolerance is one example of an enzymatic food intolerance that occurs in infants and causes gastrointestinal symptoms (such as diarrhoea and bloating). The prevalence of lactose intolerance varies dependent upon the cause, age of onset and ethnicity. In the case of both a food allergy and a food intolerance, a thorough clinical history is key to making a timely and appropriate diagnosis3,5.

IMPORTANT NOTICE: The World Health Organisation (WHO) has recommended that pregnant women and new mothers be informed on the benefits and superiority of breastfeeding – in particular the fact that it provides the best nutrition and protection from illness for babies. Mothers should be given guidance on the preparation for, and maintenance of, lactation, with special emphasis on the importance of a well-balanced diet both during pregnancy and after delivery. Unnecessary introduction of partial bottle-feeding or other foods and drinks should be discouraged since it will have a negative effect on breastfeeding. Similarly, mothers should be warned of the difficulty of reversing a decision not to breastfeed. Before advising a mother to use an infant formula, she should be advised of the social and financial implications of her decision: for example, if a baby is exclusively bottle-fed, more than one can (400 g) per week will be needed, so the family circumstances and costs should be kept in mind. Mothers should be reminded that breast milk is not only the best, but also the most economical food for babies. If a decision to use an infant formula is taken, it is important to give instructions on correct preparation methods, emphasising that unboiled water, unsterilised bottles or incorrect dilution can all lead to illness. SMA® Wysoy® milk-free formula is intended to meet the nutritional needs of babies and children who are intolerant to cows’ milk protein, lactose or sucrose. Soya infant formulae are not recommended for preterm babies or those with kidney problems, where medical guidance should always be sought. The following products must be used under medical supervision. SMA LF® is a lactose-free milk based formula for the dietary management of babies and young children who are intolerant to lactose or sucrose, or who are experiencing symptoms such as diarrhoea, tummy ache or wind caused by temporary lactose intolerance. It is suitable as the sole source of nutrition up to 6 months of age, and in conjunction with solid food up to 18 months of age. SMA LF® is not suitable for those who are allergic to cows’ milk protein, or who suffer from galactosaemia or require a galactose free diet.
